Client Information Form
To create an account, please follow these steps:
1. Register by entering your email address in the box on the right.
2. Next, enter your email address in the box on the left and click “Forgot Password.”
3. You will receive an email that will allow you to set a password for your account.
Be sure to write down your password and keep it in a safe place. BioLynceus does not have access to your password and cannot help you if you forget it.
If you have lost your password, please perform steps 2 and 3.
4. You will now be able to log into your account to provide your current shipping and billing information to BioLynceus.
